DVC TELECOM OF TEXAS will be responsible for providing telephone service to your dwelling or business. DVC will NOT be responsible for the internal wiring, jacks, individual telephone connections or any other parts, telecommunications devices or data connections. DVC will provide repair services at a nominal cost or provide a list of independent telephone repair companies. You may also choose any individual or company of your choice to provide these services at your own expense or risk. Telephone equipment of  any type is not provided as a part of this agreement. All telephone equipment or the purchase and maintenance of that equipment is the responsibility of the customer. As a customer you are not required to purchase any equipment from DVC or any authorized agent of DVC. By signing below, I stipulate that I understand the statements and agree to abide by the conditions outlined on this form and by the CUSTOMER SERVICE AGREEMENT.
